No handwritten forms will be accepted. They must be filled out, signed, scanned, and emailed to Scott Stephens.

Every year, the appropriate forms below must be filled out and submitted for every student and coach on the team.

• Student Registration & Waiver Form
  • A copy of the student's Hunter Education card must accompany the Student Form. A replacement Hunter Education card can be ordered at https://www.ilostmycard.com/

To add a new shooter or coach to your team, please fill out the appropriate form(s) below in addition to the above forms.
